UQ Health Care Homeless Service
乐信pc群新人送38the uq health care homeless service is an outreach service that plays a vital role in coordinating delivery of care for homeless people and some of the most vulnerable members in society. the service is delivered through the ozcare homeless hostel in south brisbane and the anglicare clinic in fortitude valley. partnering with multiple stakeholders, uq health care provides crisis management and coordinates care with the royal brisbane hospital homeless outreach team, princess alexandra mental health outreach team, and social services.
Refugee Health Service
the uq health care refugee health service provides health assessments, vaccination and general medical services for refugees and asylum seekers. the service is a collaboration between refugee health qld, the university of queensland school of medicine and the mater centre for primary healthcare innovation, providing valuable services to meet the complex health needs of newly arrived refugees.
SPARK Child Wellbeing Service
uq health care’s spark child wellbeing service supports children of primary school age with emotional and behavioural difficulties, academic difficulties and development disorders who require assessment, diagnosis, and treatment. the service is led by ipswich hospital paediatricians who conduct onsite specialist clinics, with physiotherapy, speech pathology, psychology, social work and occupational therapy.

UQ Health Care Aveo Durack
乐信pc群新人送38uq health care is aveo group’s primary care partner for its integrated retirement community in durack, serving 800 residents and a 123 bed high care facility.
uq health care is implementing an ‘enhanced living program’ suite of resources and services that promote healthy lifestyles for residents, and mitigate physical and mental decline. the program transforms ideas into innovative practice that delivers improved quality of care and patient satisfaction to residents of aveo retirement and aged care facilities.
the program is run in partnership with the university of queensland, which plays a key role in student placements, research and clinical innovation, as well as monthly information seminars for residents.

乐信pc群新人送38the diamantina digestive disease assessment clinic (3d clinic) is a joint initiative with princess alexandra hospital and brisbane south primary health network. the 3d clinic reduces waiting times and provides improved access to gastroenterology services at the princess alexandra hospital.
the clinic is led by a specialist gastroenterologist and uq health care general practitioners, credentialed under the princess alexandra hospital facilitated access scheme.
Metro South Complex Diabetes Service
the metro south complex diabetes service delivers advanced health services to people with complex diabetes. the service is a collaboration with princess alexandra and logan hospitals, led by specialist endocrinologists and a multidisciplinary team of diabetes nurse educators, dietitians, uq health care gps with specialised training in diabetes and other health professionals.
